Our Locations
We have 2 locations in the Birmingham area for you convenience: Crestwood and Homewood. Both locations are easily accessible, have ample parking, and provide both male and female physical therapists if you have a preference. Find out more about our locations and get directions HERE.

Clinic Logistics
- Parking: Parking is available in front of both clinics, with ample parking lot space available. For the Crestwood clinic, handicap spots are located at the end of the front row of parking spaces, directly in front of the Vineyard Food Market. In Homewood, handicap spaces are located on the side of the building; however, the spaces directly in front of our suite have no curb to navigate.
- Hours: Both clinics are by appointment only, with appointments generally available between the hours of 7:00am and 7:00pm. We do our best to accomodate your needs, and we will let you know best scheduling practices to make sure you get the appointment slots that work well for you.
- Arrival: When you enter either clinic, you will be greeted at a front desk space by our Patient Services Coordinator. There will also be chairs available in the waiting area, located directly inside the front door.
- Private Rooms: If you prefer or need a private treatment room, our Crestwood location is equipped to meet your needs.

What We Treat
Our therapists are highly trained, all having earned a Doctorate in Physical Therapy. We specialize in orthopedic physical therapy care, from generalized pain to balance issues to specific surgical rehab protocols. Each of our therapists have specific interests, and we are happy to pair you with the PT best suited and equipped to meet your individual needs. - Back/Neck Pain: We treat pain associated with a variety of spinal conditions, from stenosis to herniated discs to degenerative disc/joint disease, and more.
- Joint Pain: Our therapists are skilled at using our hands and exercise techniques to relieve pain associated with joint conditions such as osteoarthritis, rheumatoid arthritis, and other conditions that can affect the joints in your body.
- Acute or Chronic Strains and Sprains: We are skilled at rehabilitating new injuries including muscle strains and ligament sprains. We understand the tissue healing principles needed to appropriately treat both acute and chronic pains and limitations related to these injuries.

Insurance and Fees
We accept most major health insurance plans, and we are happy to verify your benefits prior to your first appointment. We are currently in-network with the plans listed below. In addition, we accept all secondary insurance plans.

If you do not see your insurance company listed here, please call for more information as you may still be eligible to use your plan at our clinic. When you call, have your insurance card available. We will likely ask for information from your card including your policy number, member ID, and the provider customer service phone number on the back of the card.
Private Pay Fees
In the current environment of rising healthcare costs, it may make sense to use our private pay options for therapy and wellness services. We strive for simplicity, transparency, and empathy with these options to make sure cost isn’t a barrier to your health.

What to Expect at Your First Visit
- Your first visit will start with the check-in process at our front desk. Upon entering the clinic, you will be greeted by our Patient Services Coordinator, who will have you complete any necessary paperwork. They will also make a copy of your driver’s license and insurance cards. You will receive an email or text with your paperwork prior to your first appointment, so you may not have additional information needed at the time of the service. Most of the paperwork is also available HERE. If you have a physician’s referral, we will also take it (or make a copy) at this time.
- After completing the check-in process, your physical therapist will come get you from the waiting room and bring you to the treatment area. You will be seated on a treatment table for the PT to begin learning more about your story. They will ask questions, discover what is most important to you, and make sure they have a thorough understanding of your specific needs. From there, they will conduct a physical exam of your issue which may consistent of a variety of quick and easy tests of motion, strength, balance, or other relevant factors.
- Your PT will then convey their findings to you, and together, you will come up with the most appropriate treatment plan. Your treatment will begin on the same day, and may consistent of hands-on manual therapy, exercise, and/or modalities as appropriate.
- Once your session is complete, your physical therapist will summarize your session, make a plan for what comes next, and tie up any loose ends. They will then lead you back to the front desk, where you will get scheduled for your appointments based on the plan discussed during your visit.
- When appropriate, you will be provided with a home exercise program after your visit via email and/or text. This will come through a program called StriveHub. You will also be provided with text or email appointment reminders, based upon your preference. These typically come to you 1-2 days before your next session.
- Feel free to ask any and all questions at any point in this process. We want to make sure you feel comfortable, heard, and are an active participant in your recovery plan.
FAQs
- When should I arrive? Please arrive 15 minutes early to your first appointment. This will give us time to complete any additional paperwork, explain your insurance benefits, and answer any questions you may have. For follow up visits, it is not necessary to arrive early. We do run on time, so please let us know if you will be late so we can make the best plan.
- What should I wear? Comfortable clothes that allow you to move are best. Also consider the part of your body receiving treatment when choosing your clothing, as certain areas may need to be exposed. (For example, please wear pants or leggings that can be rolled up if we are working on your calf. If you are receiving dry needling to your upper back, a sports bra may be best. Consider an elastic waist band if your hip may need to be exposed.)
- What should I bring with me? Please bring your idenification and insurance cards to your first visit. Also bring your physician’s referral if you have it. However, in most cases, a doctor’s referral is not needed to begin PT. There is no need to bring any exercise bands, equipment, etc with you.
- Do I need a referral from my doctor? No, you do not need a physician’s referral to get started with physical therapy in most cases. If it is required for your insurance, we will let you know at the time of scheduling. We will also work with you to get a referral if it becomes appropriate during the course of your treatment.
- How long does my PT session last? Your first session will typically last between 45 minutes – 1 hour with your PT, with additional time needed to complete paperwork and schedule appointments. Follow up appointments will last for 45 minutes with your therapist, with extra time needed for any modalities such as ice, heat, compression, or dry needling. If you are limited in your schedule, let us know at the beginning of your session so we can make the most of the time we have together.
- Will I owe anything at the time of service? Yes, copays and coinsurance costs are collected at the time of service. If you need special accomodations, please make sure to let us know so that we can formulate the best plan for you.
- What is your cancellation policy? We ask that you give us 24 hours notice if you need to cancel or reschedule an appointment. Your appointment time is reserved solely for you; therefore a $50 fee may apply for late cancellations or no-shows.
